Carer Support Programs - Dementia provide support and assistance to carers of people with dementia.

 

Carer Support - Dementia and Behaviour

 

This program provides intensive support to carers of people with dementia experiencing behaviours of concern.

These programs are available to carers

  • Who live in the Bayside, Cardinia, Casey, Frankston, Glen Eira, Greater Dandenong, Kingston, Mornington Peninsula, Port Phillip or Stonnington local government areas, and
  • Who have been providing care that is likely to be ongoing for six months or more.
Carer Support Workers assist carers to find the balance between caring for others and caring for themselves. They can
  • Assist carers to plan their short and long term care and support needs
  • Provide carers with emotional support and help identifying ways to improve their wellbeing
  • Provide information about the range of aged/dementia services available
  • Discuss strategies to reduce behaviours of concern and improve quality of life
  • Provide carers with information about dementia and discuss how this relates to their experience, and
  • Connect carers and the people they care for to appropriate local services.

The Weekenders

 

This program supports carers of people who have dementia and are over 65 years of age, who have predominantly high care needs on Saturdays and low care needs on Sundays.

 

Outings and site-based activities are provided from the Napier Street Day Centre in South Melbourne. Lunch and morning/afternoon teas are provided. Transport to and from the Napier Street Day Centre is available for participants who live close to the Centre, within the times available.

To Steki Tis Haras (Greek Host Home Program)

 

This program supports carers of people of Greek descent who have dementia and low care needs.

 

The service is delivered in partnership with Fronditha Care. Culturally specific daytime respite (for up to four people) is provided once a week in the home of a Greek-speaking Host in Bentleigh East. Transport to and from the Host home, with Greek-style meals and outings of interest provided.

Small Group Outings

 

This program supports carers of people who are more active, with early stage dementia.

 

The service is delivered in partnership with the Central Bayside Community Health Service.

 

Supervised outings are arranged to cafes and locations of interest for people who want to socialise on a regular basis, but may not be suited to formal groups. Availability is limited.

 

  • Give It A Go

 

This program caters for people under 70 years of age who are experiencing early stages of dementia.

 

This group meets weekly, on Wednesdays, for social outings where other aged activities are not appropriate.

  • Let’s Go For Coffee

 

This group meets weekly on Thursdays, for social outings where other aged activities are not appropriate.
